Job Description

We have an exciting opportunity for two full-time Care Managers to join our Out of Hospital Care (OHC) program in Bella Vista on maximum-term contracts, with potential for extension.

  • One contract through to June 2026
  • One contract through to January 2027

About the role

As a Care Manager, you’ll work with clients and stakeholders to assess, plan and manage services that enable people to safely return home from hospital. You’ll ensure clients have the right supports in place for independence, choice and control, while also promoting the benefits of Care Connect’s wider services. These roles are based out of our Bella Vista office: 1 covers the Metro Sydney area & the other covers SE Sydney. Depending on location this can be a hybrid or fully remote role, with requirements to attend the Bella Vista office on occasion for training or meetings.

In this role, you will:

  • Review referral and intake information, completing client assessments (face-to-face or via phone) within agreed timeframes and KPIs
  • Develop person-centred care plans – identify needs, explain rights & responsibilities, and set client expectations collaboratively
  • Build strong referral networks and connect clients with local resources aligned to their care plans and budgets
  • Provide service excellence across all stages of the client journey

About You…

What you’ll bring:

  • Strong experience in case management or client management
  • Diploma or degree in Health/Allied Health or related fields OR 6 years + experience
  • Strong communication, advocacy and interpersonal skills
  • Proven time management, prioritisation and budget management skills
  • Current Australian driver’s licence and reliable vehicle
  • Proficiency in Microsoft Office and willingness to learn client database systems
  • Immunisation record in line with NSW Health policy (including COVID-19), or willingness to obtain prior to employment

 

To be considered for this role you must have full Australian working rights, current Police & Working With Children Checks – we can arrange police checks for successful candidates. Our process also includes psychometric assessment.
