Care Connect is delivering meaningful transformation — and we’re looking for an experienced Project Manager to help drive this change.
As part of our Project Management Office (PMO), you will lead the delivery of strategic projects and service improvements that support how we operate, grow and better serve our clients.
This is a key delivery role within our Strategy & Delivery team, offering the opportunity to work closely with senior leaders and influence high-impact organisational initiatives.

About the Role
Reporting to the PMO Manager, the Project Manager is responsible for delivering a portfolio of strategic and operational projects aligned to organisational priorities.
You will lead projects end-to-end — from defining scope and requirements through to implementation and transition into business-as-usual.
This role is not just about delivery.
It is about ensuring projects are well-defined, effectively governed, and deliver meaningful, lasting outcomes for the organisation and the people we support.
You will:
- Lead the full lifecycle delivery of projects (scope, schedule, budget and outcomes)
- Translate business needs into clear project plans, requirements and deliverables
- Monitor performance, risks and financials, ensuring projects stay on track
- Develop high-quality project artefacts including plans, RAID logs and reports
- Facilitate workshops and engage stakeholders to gather and validate requirements
- Provide clear analysis, insights and recommendations to support decision-making
- Ensure strong governance, compliance and reporting across all projects
- Support change management and transition to business-as-usual operations
- Contribute to continuous improvement of PMO frameworks, tools and practices

You are a structured and collaborative project professional who enjoys delivering outcomes and working across diverse stakeholder groups.
You are likely someone who:
- Has hands-on experience delivering projects across multiple workstreams
- Builds strong relationships and communicates with clarity and confidence
- Can break down complex problems into clear, actionable solutions
- Is highly organised and able to manage competing priorities
- Takes a proactive, analytical and solutions-focused approach
- Is comfortable working in a dynamic, evolving environment
Key Capabilities
- End-to-end project management across varying scale and complexity
- Strong stakeholder engagement and facilitation skills
- Project governance, risk and reporting expertise
- Financial management including budgeting and forecasting
- Analytical thinking and problem-solving capability
- Experience using tools such as MS Project, Jira, Excel and Confluence
- Ability to support change and continuous improvement initiatives
Qualifications & Experience:
- Tertiary qualification in Business, IT, Health Informatics or similar (or equivalent experience)
- 3–5+ years’ experience in project delivery roles
- Experience in healthcare, aged care or public sector environments (highly regarded)
- Project management or change certification (Agile, PRINCE2, PMBOK, PROSCI) desirable
